171192: Time­out of ac­tive trans­ac­tion: log co­or­di­na­tor ID and stack trace of ac­tive thread

We now log more con­text in case of time­outs: the co­or­di­na­tor ID and the stack trace of the ap­pli­ca­tion thread - mak­ing it eas­i­er to de­tect block­ing sit­u­a­tions that make the trans­ac­tion time out.
